Website intake into Cliniko for a physiotherapy clinic
A physiotherapy clinic's website has many intake forms, and its patients are managed in Cliniko. Working in the Make account of the clinic's digital agency, Skofi built one scenario that takes every form submission and routes it into Cliniko through a single router tree.
- runs in a month, no errors
- 20
- scenario for every form
- 1
- branches in the router tree
- ~17
The organisation
The clinic's website, run by a digital agency, collects enquiries and patient details through Gravity Forms, with different forms for different services and situations. The clinic runs its appointments and patient records in Cliniko, a practice-management system built for allied health.
Every submission needed to reach Cliniko in the right shape for its form, without reception retyping health information and without a separate automation for each form to maintain.
The build lives in the agency's Make account, which holds only this scenario. No patient information appears on this site.
The outcome
20 runs between 19 September and 21 October 2026, all successful, using 80 credits.
Switched on since September 2025 and last changed in August 2026, with no errors in its visible history.
Where it stops
One scenario carries every form, so a fault in it affects all of them, and it has no error route beyond Make's default. The run figures cover one month; earlier history is outside Make's retention window.
